We are seeking a part-time, 24-hour per week, Utilization Management Nurse. From our office in Brecksville, this position provides service coordination and utilization management for patients throughout Ohio receiving home health and infusion services.   
Responsibilities include:
    * Reviews referral data for assessment and planning of a comprehensive plan of care to meet the patient's needs.
    * Monitors case needs on an ongoing basis, obtaining clinical updates and insurance authorizations in a timely manner.
    * Clinically evaluates the plan of care as the case evolves and works collaboratively with the agency and insurance company to modify care approach as needed.  
    * Documents clinical information accurately and concisely.
Requirements include:
    * RN or LPN with a current OH license
    * 2 years of clinical experience in an acute care setting
    * Previous home health care, infusion therapy or insurance/ managed care experience is preferred
    * CCM certification a plus
    * Outstanding communication skills.
    * Excellent organization reflected in work
    * Intermediate computer skills, including experience with windows, Microsoft office and the Internet.
