Job Summary    The Clinical Nurse II will work independently and as part of a team to provide support to multiple faculty clinicians who work in a collaborative program with the Correctional Medicine Consultation Network (CMCN) in the Department of Family and Community Medicine, UCSF, and the State of California Department of Corrections and Rehabilitation (CDCR) to improve the quality of health care within the state’s 32 correctional facilities. Duties include: working independently and collaborating with a multidisciplinary team to coordinate and implement the clinical care of patients, utilizing the nursing process to provide safe, therapeutic care management of patients with HIV disease, Hepatitis C and Transgenders who reside in prisons but who are seen by UCSF experts through the use of telemedicine and on site visit protocols; managing of patients in conjunction with the medical providers, social workers and other clinical providers (care coordination, triage and case management); tracking telemedicine patients using Patient Tracking System and following up on lab and diagnostic procedures in conjunction with UCSF providers; facilitating support groups for pre-, on- and post-Hepatitis C patients; collaborating with CDCR nursing and provider staff to ensure continuity and quality of care for patients seen in telemedicine clinics; triage of patients referred to the CMCN HIV program; onsite work at various prison sites is required; and perform other duties as assigned. Note: There are four positions available.
Required Qualifications    Possession of the legal requirements to practice as a Registered Nurse as determined by the California Board of Registered Nursing and six months of clinical experience; 4-5 years of experience in care coordination, triage and case management in an outpatient clinic setting; HIV and/or Hepatitis C knowledge including disease progression, appropriate treatments, complications, prophylaxis medications, basic antiretroviral management and drug interaction; experience working with incarcerated or underserved populations; must pass any requested security checks by the State of California to include, but not limited to, fingerprinting and a full background check.
Preferred Qualifications    BS in Nursing or health related bachelor’s degree; experience in managing telemedicine clinics; support group facilitation experience.
