Job Description Assesses information from families, hospitals, physicians, nursing homes, community agencies and other referral sources pertinent to patient admission services.
Provides guidance and instruction to DME/RT and IV clients regarding supplies, equipment and services and assists in arranging delivery of equipment and supplies.
Collects and records medical, social and reimbursement data pertinent to patient admission to initiate home care, hospice, DME/RT and supplies and service.
Responds to inquiries regarding VNA services and programs. Explains those programs and services, in addition to, admission policies and/or referring requests to other community agencies when appropriate.
Functions as a resource to VNA non-clinical customer service staff on patient care and coordination issues.
Coordinates and relays referral information to appropriate departments/divisions within the organization.
Interprets home care and IV pharmacy insurance contract information and advises customers, referral sources or internal staff.
Assists in maintenance of accurate records of patient admission and discharge information via computer and manual systems.
Collects statistics pertinent to Intake/Referral Coordination.
Participates in chart audits, continuous quality improvement activities and in-services.
General Qualifications Current licensure as a Registered Nurse (RN) granted by the State of Wisconsin.
Minimum two (2) years of experience as a RN
Strong verbal and written communication skills.
Excellent interpersonal skills necessary to interact with internal and external clients.
Thorough understanding of home care reimbursement including Medicare, Medicaid and private insurance.
Must be detail-oriented and have excellent computer and organizational skills.
Desired Qualifications Experience in home health care, case management or discharge planning preferred.
