R.?N.? Hospice Case Manager Full Time: Responsibilities include: visiting patient’s in their residences, performing initial and ongoing assessments, performing supervisory visits for hospice aides, maintaining clinical documentation, communicating with patient families, physicians, and other health care facilities.? Hospice experience required.
Admissions Coordinator Full Time – RN BSN Required: Responsibilities include: initial referral and admissions, intake, obtaining all pertinent medical information regarding patient’s hospice diagnosis, working with State & Federal regulations, doctor’s orders, eligible insurance processing, etc.?, explaining hospice services to patients and families.? Hospice intake and admissions experience required.
Director of Social Work Full Time – LCSW or MSW: Responsibilities include: providing individual and family counseling to hospice patients and families in their place of residence, supervision of hospice social workers, oversight of hospice volunteer, bereavement programs, and hospice chaplains.
