    * Job Summary: The LVN, Family Care Services, works under the supervision of a Registered Nurse (RN) to coordinate, organize, and monitor the developmentally appropriate and age-specific (infant, toddler, preschooler, school-age, adolescent, young adult, middle adult, later adult) direct nursing care to patients in the home environment; serves patient population requiring constant attendance and care during assigned shift, e.g., tracheostomy, complex ventilator management; high risk to fall or choke; utilizes interpersonal skills to maximize customer service in a manner that supports the I CARE initiative, excellence in customer service, safe patient care and professionalism; contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population; performs other duties as assigned. Job Specifications: Current and unrestricted CA. LVN license, current BLS, 1 year experience as a LVN required; clinical experience in medical/surgical nursing preferred. Ca. IV certification for designated areas preferred. Other certifications as identified in department-specific Plan for Providing Care. Able to: read; write legibly; speak in English with professional quality; assemble, troubleshoot, and calibrate patient care equipment; perform technical patient care activities as required; use computer and software programs necessary to the position. Able to lift at least 50 pounds repetitively to move, transport patients, equipment and supplies; lift at least 10 pounds above shoulder level.
